2) You need to complete a Scarva Street Health Questionnaire
3) You need ID.

1) Medical Card
If you do not have a medical card you need to complete either
+ a HS200 – if your previous address was in the UK OR
+ a HSC/R1 – if your previous address was outside the UK, even if you were previously registered in the UK
2) Scarva Street Surgery Health Questionnaire
This needs to be completed fully &
+ If you are on any regular medication we need a list from your previous GP or the right hand side copy of your prescription
+ If a child under 5, we need a copy of their vaccination history from their red book.
3) ID
You will need Photo ID, proof of address & right to reside in NI.
